Account Request
This form is the first step to enrolling your new student online. Complete it to request an account that you will use to log in to a secure system.
Complete required responses (*) to request an account to enroll your student(s).
Use an email account to have password sent to your email account.
If you don't have an email, check box "I don't have an email" to create your own username. Use a unique username such as your 10 digit phone number. A password will be displayed after you submit request.
Write down your username and password to continue working on saved applications.
